Notice of Infringement – Claim

  4. Contact information of the complaining party, including name, address, email, phone number, and fax number;
  5. A statement confirming unauthorized use of the material;
  6. A statement affirming the accuracy of the notification and the authority to act on behalf of the copyright owner.

Title 17 USC §512(f) imposes penalties for misrepresentation in infringement notifications.

Counter Notification – Restoration of Material

  3. A statement expressing your belief that the removal was erroneous.
  4. Your contact details and consent to the jurisdiction of the court.
