Notice of Infringement – Claim

- Contact information of the complaining party, including name, address, email, phone number, and fax number;
- A statement confirming unauthorized use of the material;

Counter Notification – Restoration of Material

- Your signature.
- Description of the removed material and its original location.
- A statement expressing your belief that the removal was erroneous.
- Your contact details and consent to the jurisdiction of the court.
